Capitol Bridge is seeking a full-time Case Manager to join our team in District of Columbia (DC). Since 2012, our employees have been entrusted with helping the business of government work better for our customers. We are looking for (2) two experienced Case Managers(s) to review and evaluate workers' compensation proposals based on a set of criteria.
This is a full-time position, Monday - Friday, 8:00am - 5:00pm.

Base Rate: $60,000

Duties
Act as the ongoing point of contact for program participants (up to 20 per cohort, 80 total).
Provide comprehensive case management services including intake essment, progress oversight, regular check-ins, advocacy and referrals to support services.
Track progress in real-time to ensure attendance and success.
Mitigate enrollment and participation challenges via scheduling, resource referrals, or similar accommodations.
Maintain program participant case files by collecting, verifying, and updating relevant information.
Work with support team and other key stakeholders to develop and maintain administrative doents, policies, and procedures.
ist with reporting on program performance measurements and other relevant data.
Education and Qualifications:
Bachelor’s degree required; master's preferred.
Degree in Social Work, Psychology, Counseling, or other related social service field preferred.
Licensure (LMSW or LBSW), preferred.
Minimum of two (2) years direct service/case management experience
Ability to respond to resolve crisis situations.
Strong knowledge of community resources and referral process.
Knowledge of substance abuse/recovery, family systems, medical and mental health treatment preferred.
Ability to interact and work collaboratively in a flexible work environment with a variety of constituents and colleagues.
