Physical Therapist Home Health

• Meets the patients goals and needs and provides quality care by assessing and interpreting evaluations and test results; determining physical therapy treatment plans in consultation with physicians or by
  
• Helps patient accomplish treatment plan and accept therapeutic devices by administering manual
  
• Evaluates effects of physical therapy treatments and fit of prosthetic and orthotic
  
• Completes discharge planning by consulting with physicians, nurses, social workers, and other health care
  
• Assures continuation of therapeutic plan following discharge by designing home exercise
  
• Documents patient care services by charting in patient and department
  
• Protects patients and employees by adhering to infection-control policies and
  
• Contributes to team effort by accomplishing related results as
  

Requirements of Home Health Physical Therapist

• Graduate of an approved Physical Therapy
  
• Current license to practice in Notice: All licensure and certification must be obtained according to the current Licensure / Certification Policy and
  
• Ability to work with clients of all
  
• BLS certification
  
• Knowledge in current treatment modalities and their safe use in the
  
• An awareness regarding the Medicare regulations for coverage of physical therapy in the
