JOB DESCRIPTION:

ER-1 Description

Coverage Type:

114 hospital bed capacity

36 hours of physician coverage daily 27k volume

Medium to high acuity Cutting edge technology

Intensivist coverage daily 8 am to 4pm daily

36 Hours of physician coverage

Average daily census PTS per hour

ER-2 Description

Closed ICU

No Procedures Required

SUBMISSION REQUIREMENTS

Provider must have a clean file and 7 plus shifts available per

All candidates must have met the minimum shift availability listed on the

CV must have all dates in MM/YY to MM/YY format

Work History must be in Sequential order with most resent-on top

Any Gaps over 30 days must be explained on the CV

EPA (Please use the attached It s also in Trio under your Name & then click on Help)

All Licenses, Boards and Certs with expiration dates must be listed on CV

Anyone with malpractice must provide a NPDB

Anyone with legal issues must provide supporting documents

Availability

1st possible date they would be available?

How many shifts would they be able for per month?

What month is their schedule completely open?

Any shift preferences (Day, Night, Blocks etc)?
