Nurse Case Manager Salary Range:

Nurse Case Manager Duties:

• Nurse Case Manager interviews inpatients, outpatients and families to access discharge planning
  
• Provides direct intervention to patients as individual needs are identified along with psychosocial support for patient and families
  
• Established discharge priorities and formulates a functional individualized discharge plan with patient/families
  
• Makes appropriate referrals by utilizing available hospital and community
  
• Assists in coordinating the patients, care with other involved departments in order to facilitate the patients discharge
  
• Collects necessary admission and continued stay information on all patients and when necessary communicates the information to the appropriate review organizations pursuant to hospital policy and
  
• Conducts chart reviews and presents outlying cases to UR Committee
  
• Concurrent documentation reviews will be performed throughout the patients stay to ensure ongoing medical necessity and that treatment is provided at the correct level of care
  
• Maintains a positive work environment and supports the overall team
  
• Nurse Case Manager is able to work independently and prioritize

• Schedule 4 days/week, 8 hour shifts, day shift, M-F
  
• No Holidays, Weekends, or Call required!
  

Nurse Case Manager Qualifications:

• Must have RN or LPN license in state of Colorado
  
• Three year of nursing experience preferred in hospital-based setting
  
• Working knowledge of CMS Standards
  
• Team oriented and self-started with the ability to organize work, meet deadlines, and and work independently with minimum supervision
  
• Experience with EMR/Meditech systems
