Case Manager

• Assesses patients’ needs relative to discharge planning and utilizes appropriate resources in a timely fashion.
• Serves as a liaison between the patient, physician and payer along the continuum of care
• Determines the level of care appropriate for patient upon discharge
• Ensures the completion of documentation related to discharge planning
• Partners with physicians and nurses to ensure a comprehensive and coordinated plan of care
• Works with physicians and nurses to correct utilization deficiencies and delays
• Provides patient care information as requested to contracted insurers and/or reimbursement entities within the guidelines of the confidentiality information policy
• Acts as an advocate on behalf of the patient to protect the patient’s health, safety and rights
• Determines the level of care upon request for admission utilizing evidence-based criteria

EXPERIENCE REQUIREMENTS

• Minimum of five (5) years experience post graduate is required
• Must have excellent communication skills and the ability to produce reports in a timely fashion
• International accreditation experience is preferred
• Evidence of progressive leadership experience is preferred
• Knowledge of pre and post acute venues of care
• Demonstrated ability to appropriately interact with physicians, nurses, patients and their families
• Excellent interpersonal skills
• Proficient in the use of PCs and spreadsheets
• Strong organizational and time management skills
• Strong oral and written communication skills
• Ability to speak and write in English is required
• Ability to speak and write in both English and Arabic is preferred
