Requirements
Qualified Candidate:
- CMA/LPN/ RN certificate /degree
- Empathetic and active listener
- Effective time management skills
- Strong communication and interpersonal skill - both written and verbal
Care Coordinators will -
- Review patient records to ensure that information is accurate.
- Ensure that patient records/documents accurately reflect the current conditions/treatment requirements of each patient.
- Provide preventative care by identifying care gaps.
