Position Purpose

The Nurse Case Manager provides intensive nurse case management to men living with HIV who receive their medical care at our ; The Nurse Case Manager promotes adherence to medical care and collaborates with other programs within the company and with local service providers for the benefit of patients in both

**This is a Full-Time Monday-Friday position with no nights, weekends, on-call or holiday **

Essential Functions

  • Provide Nurse Case Management for men accessing the company who have complex medical needs by identifying patient/family health needs, and then developing, implementing, and monitoring health care plans to meet patient/family health This includes facilitating access to benefit and entitlement programs and ensuring patients adhere to care from an HIV specialist as well as to other needed care such as primary care, gynecological evaluations, and mental health
  • Provide education to patients on HIV disease, other health conditions, medical care issues, lab results and adherence strategies to prescribed ; Provide education to family and support persons, as well as to area professionals, concerning the needs of people living with HIV ; Inform patients of the services available through the company and in the community, and link patients to them as
  • Collaborate extensively with the companys Dental, Behavioral Health, Case Management and Prevention staff, to ensure that patients and clients obtain appropriate and timely access to all needed care and to maximize adherence to treatment
  • Establish a working relationship with appropriate community resources, including those providing medical care, mental health care, dental care, AODA treatment, and support services to underserved and target
  • Maintain knowledge and skills relevant to HIV/AIDS, with emphasis on issues specific to men living with HIV, through self-study and through participation in trainings, workshops, and in-
  • Maintain accurate, up to date patient records necessary for patient and program This includes documentation in Epic, the electronic medical record for clinic Assist with all documentation and information needed for grant
  • Contribute as an active member of the Company, as well as of the Primary Care Support Network, including participating in Quality initiatives, staff meetings, multidisciplinary staffing s, in-service trainings and workshops, department or agency planning activities; assisting in special projects; and providing support, peer mentoring and training to

Qualifications

  • Licensed Registered Nurse; Certification CPR-BLS preferred
  • 2+ year of case management experience in a community, public health, or mental health setting
  • 2+ year or greater experience in medical/surgical nursing, primary care, or specialty office
  • Thorough knowledge of nursing practice standards
  • Knowledge of and sensitivity to the HIV/AIDS patient population, and/or an eagerness to learn about this community
  • Valid Missouri Driver License, insurable driving record, and access to a reliable insured vehicle
  • Willingness to travel is required; ability to be flexible in hours, including some evening hours
  • Exemplary customer service skills
  • Excellent communication skills
  • Effective organizational skills
  • Able to work with a diverse population
  • Working knowledge of Spanish language (bilingual) is a plus
