Position Description:
The Counselor will provide support and resources to clients in LTC/skilled nursing facilities. Additionally, Counselor will directly counsel patients, consisting of individual, and group services, including individualized treatment plans, counseling, and aftercare services rounding to Nursing Homes, and doing follow-ups.
Requirements:
- Licensed as an LCSW
- Dynamic and personable personality
Schedule:
Full time preferable - Part-time option available
