Human Services Practitioner (Licensed Clinical Social Worker - Outpatient Therapist)

RESPONSIBILITIES:

• Provides direct outpatient care to adult, child and adolescent patients utilizing evidence-based therapeutic modalities. Acts as an independent licensed provider in a competent, capable and professional manner that exemplifies current standard of care and practice in therapy. Develops and updates a treatment plan to help patients meet their treatment goals following current standards of practice.
• Performs assessments on new and returning patients to diagnose and treat mental health and psychological disorders; Assesses emotional, psychological, and developmental status of patients utilizing a biopsychosocial approach.
• Communicates and coordinates care with other internal and external providers and care teams as necessary. Documents in electronic medical record in a timely manner following standards of care. Reviews and responds to tasks and messages in the electronic medical record in a timely manner following standards of care.
• Performs other clinical duties as needed and assigned.
  

POSITION QUALIFICATIONS:

MINIMUM:

• Master's degree in social work;
• State of Florida professional license (LCSW) in good standing is required;
• Must be able to obtain and maintain credentialing with USF Health
  

PREFERRED:

• Outpatient therapy experience.
