Key Responsibilities
1. Patient Assessment & Clinical Management
- Conduct comprehensive medical assessments and develop individualized treatment plans for home care patients.
- Diagnose and manage acute and chronic medical conditions.
- Monitor patients recovering from critical illnesses or post-ICU hospitalization.
3. Treatment & Medication Management
- Prescribe medications therapies and diagnostic tests as required.
- Review and adjust treatment plans based on patient progress and response.
- Ensure safe medication administration and monitor for adverse reactions.
4. Home Visits & Patient Care Coordination
- Conduct regularhome visitsto assess patient condition and treatment effectiveness.
- Coordinate withhome healthcare nurses physiotherapists and caregiversto ensure continuity of care.
- Liaise with hospitals and specialists when escalation of care is required.
5. Emergency Response & Clinical Decision Making
- Identify early signs of medical deterioration and take immediate action.
- Provide urgent medical interventions when necessary before hospital transfer.
- Guide caregivers and nurses on emergency response procedures.
6. Patient & Family Education
- Educate patients and families on disease management medication adherence and lifestyle modifications.
- Provide guidance on wound care nutrition infection prevention and rehabilitation.
7. Documentation & Compliance
- Maintain accurate medical records and treatment documentation.
- Ensure compliance withhome healthcare regulations and clinical protocols.
- Prepare clinical reports and follow regulatory standards.
