Job Overview:
GluCare Integrated Diabetes Center is seeking a dedicated DHA Nurse to join our clinical team in delivering exceptional patient care. The ideal candidate is compassionate adaptable and committed to providing highquality nursing services in a modern fastpaced environment. Experience in chronic disease management patient education and digital tools is a plus.
Key Responsibilities:
Patient Care Support:
- Provide direct patient care including vital sign checks blood glucose monitoring and patient observations.
- Educate patients on diabetes management including lifestyle tips and monitoring instructions ensuring compliance with care plans.
Clinical Procedures Assistance:
- Prepare examination rooms and medical equipment ensuring cleanliness and adherence to infection control standards.
- Assist in the preparation and handling of equipment for procedures wound care and specimen collection.
Patient Education and Communication:
- Communicate effectively with patients providing empathy and clarity to address their questions and concerns.
- Reinforce health education provided by physicians encouraging selfcare practices and adherence to treatment plans.
Documentation and Reporting:
- Accurately record and update patient information treatment plans and observations in electronic medical records (EMR).
- Report any changes in patient condition to the supervising nurse or physician promptly.
Operational and Safety Compliance:
- Adhere to all DHA regulations nursing guidelines and GluCare protocols to ensure the safety and wellbeing of patients and staff.
- Participate in ongoing training and development programs to stay current with best practices in diabetes care and nursing support.
RequirementsQualifications:
- Valid DHA Nurse License A MUST
- Minimum of 12 years of experience in a clinical setting preferably in endocrinology diabetes care or chronic disease management.
- Knowledge of diabetes management and patient education in lifestylerelated chronic conditions is preferred.
- Strong communication and interpersonal skills with a patientcentric approach.
- Basic proficiency in electronic medical records (EMR) systems is preferred.
