One Community Health

One Community Health is a primary healthcare and specialty care provider dedicated to improving the health and well-being of our community. Since our beginning in 1989, it has been our mission to create a healthier Sacramento by increasing access to care in our community, regardless of age, gender, ethnicity, orientation, or ability to pay. We want our patients to feel comfortable and well cared for. But more than that, we want them to be a partner in their care. We want patients to understand, to ask questions, and to be aware that they can work in partnership with the staff to make life choices that will lead to improved health. We use a medical home model of care, including preventive care, routine checkups, immunizations, and management of acute and chronic illnesses. The patient centered medical home is a model of care that emphasizes care coordination and communication to transform primary care into “what you want it to be.”
