Job Description
Roles & Responsibilities
Provide direct patient care in the home setting, including administering medications, monitoring vital signs, and assisting with activities of daily living.
Assess patients' health status and develop individualized care plans based on their needs and goals.
Collaborate with physicians and other healthcare professionals to coordinate and manage patient care.
Educate patients and their families on healthcare topics and self-care techniques.
Monitor and document patients' progress and communicate any changes or concerns to the healthcare team.
Administer and manage medications, including IV therapy and pain management.
Ensure the safety and comfort of patients in their home environment.
