Home Care Nurse

Key Responsibilities:

• Patient Assessment: Conduct comprehensive assessments of patients in their homes including evaluating their medical condition environment and care needs.
  
• Care Planning: Develop individualized care plans in collaboration with physicians therapists and other healthcare professionals tailored to each patient s unique needs.
  
• Nursing Care: Provide a wide range of nursing services including wound care medication administration monitoring vital signs managing chronic conditions and assisting with activities of daily living (ADLs).
  
• Patient Education: Educate patients and their families on managing their health including medication management dietary guidelines and proper use of medical equipment.
  
• Health Monitoring: Regularly monitor patients health status and update care plans as needed. Report any significant changes in patients conditions to the supervising physician.
  
• Coordination of Care: Coordinate with other healthcare professionals to ensure seamless care including arranging followup appointments lab tests and specialist consultations.
  
• Documentation: Maintain accurate and uptodate patient records including care plans treatment notes and any changes in the patient s condition.
  
• Patient Advocacy: Act as an advocate for patients ensuring they receive the necessary care and support while respecting their autonomy and preferences.
  
• Compliance: Adhere to all hospital policies protocols and regulatory standards including those related to patient confidentiality infection control and safety.
  

Qualifications:

• Education: Bachelor s degree in Nursing from an accredited institution.
  
• Licensure: Must hold a valid nursing license recognized by Dubai Health Authority (DHA).
  
• Experience: Minimum of 23 years of nursing experience with at least 1 year in a home care or community health setting.
  
• Skills:
  • Strong clinical skills and the ability to provide highquality nursing care in a home setting.
    
  • Excellent communication and interpersonal skills with the ability to build rapport with patients and families.
    
  • Ability to work independently and make informed decisions in the field.
    
  • Cultural sensitivity and the ability to work in a multicultural environment.
    
• Languages: Proficiency in English is required; knowledge of Arabic is an advantage.
  
• Driver s License: A valid UAE driver s license is required as the role involves traveling to patients homes.
  

Working Conditions:

• Hours: Fulltime position with flexible working hours depending on patient needs. May include evening or weekend visits.
