Intake Operations Coordinator

Duties

Receive and develop referrals from all Liaisons.

Review all intakes for services requested working closely with RN/LPN.

Maintain current system of tracking clients who have been admitted to the hospital and are pending hospital discharge.

Assist in coordination of services to ensure a smooth, effective start of care.

Respond timely to all correspondence received from internal and external sources.

Input initial admission and financial information into the computer system.

Verify physicians license.

Maintain our current referral tracking system to ensure all patients are seen timely.

Make and distribute referral copies.

Assist Admissions Coordinator as needed.

Other Duties

Performs assigned work safely, adhering to established departmental safety rules and practices; reports to supervisor, in a timely manner, any unsafe activities, conditions, hazards, or safety violations that may cause injury to oneself, other employees, patients and visitors.

Qualification

High School diploma required; 2 years additional education preferred

Computer/typing Experience Required

Home Care experience preferred (3-5 years)
