Responsibilities:
Patient Assessment and Counseling:
- Conduct comprehensive nutritional assessments for patients with diabetes considering their medical history lifestyle and dietary habits.
- Develop personalized nutrition plans tailored to each patients specific needs goals and preferences.
- Provide one-on-one counseling sessions to educate patients on diabetes management healthy eating habits portion control and carbohydrate counting.
Nutritional Education and Support:
- Conduct group education sessions and workshops on topics such as meal planning blood sugar control and weight management.
- Create educational materials including brochures handouts and online resources to support patient education and self-management.
- Collaborate with other healthcare professionals including endocrinologists nurses and diabetes educators to provide comprehensive care to patients.
Monitoring and Evaluation:
- Monitor patients progress and adjust nutrition plans as needed to achieve desired health outcomes.
- Track and document patients nutritional intake weight blood sugar levels and other relevant health indicators.
- Provide ongoing support and motivation to help patients adhere to their nutrition plans and make sustainable lifestyle changes.
Research and Best Practices:
- Stay current with the latest research and advancements in diabetes nutrition and care.
- Implement evidence-based practices and guidelines in all nutritional counseling and education activities.
- Participate in continuing education opportunities to enhance professional knowledge and skills.
Community Outreach:
- Participate in community outreach programs to promote diabetes awareness and prevention.
- Conduct presentations and seminars for community groups schools and organizations to educate the public on diabetes management and healthy eating.
RequirementsQualifications:
Education:
- Bachelors degree in Nutrition Dietetics or a related field (required).
- Masters degree in Nutrition or related field (preferred).
Certification and Licensure:
- Registered Dietitian (RD) or Registered Dietitian Nutritionist (RDN) credential (required).
- Licensed Dietitian/Nutritionist (LDN) in the state of insert state (if applicable).
- Certified Diabetes Educator (CDE) or Board Certified-Advanced Diabetes Management (BC-ADM) (preferred).
Experience:
- Experience in clinical nutrition preferably with a focus on diabetes care.
- Experience in patient counseling group education and creating individualized nutrition plans.
Skills:
- Strong knowledge of diabetes management and nutrition therapy.
- Excellent communication and interpersonal skills with the ability to effectively educate and motivate patients.
- Detail-oriented and organized with the ability to manage multiple patients and tasks simultaneously.
- Proficiency in using electronic health records (EHR) and nutrition software.
