We are seeking a Nurse Practitioner or Physician Assistant to see geriatric patients in a Nursing Home setting in the area of Queens, NY.

Duties will include:

• Managing all Primary Care related consults.
  
• History and physical exam
  
• Medication review
  
• Prescribe medication and drugs.
  
• Check in with patients who have acute issues and establish a follow-up plan.
  

Requirements:

Valid NP/PA license

Geriatric Experience preferred
