• Deliver evidence-based, timely care in a manner that reduces avoidable hospitalizations, maximizes quality of life, and puts patient health and satisfaction
  
• Conduct assessments, which includes comprehensive annual wellness exams on (Assessments will occur )
  
• Prescribe medications, order tests, and collaborate with patient s
  
• Document items such as: chief complaint, all applicable diagnosis, past medical, family, and social history, review of systems, examinations, medications, allergies, assessment, and
  
• Collaborate with care management RNs and SWs to ensure consistent, highly satisfying patient experience, accurate documentation, and effective care Complete all documentation and paperwork in a timely
  
• Maintain quality of care and productivity standards as defined by the
  
• Perform effectively, as reflected by improved patient quality outcomes, which will be measured and reported
  

Requirements

• Active and unrestricted Maryland Nurse Practitioner license
  
• Board Certified by ANCC or AANP
  
• Current and unrestricted DEA Certification
  
• 2+ years of NP experience
  
• Experience practicing in managed care, value-based context required
  
• Experience conducting annual wellness visits, comprehensive visits and appropriately charting HCCs
  
• General knowledge of risk adjustment
  
• Experience in kidney disease and control of related co-morbidities (HTN, Diabetes, CHF) preferred
  
• Experience in end-of-life discussions, palliative care and hospice preferred
  
• Ability to work without direct in-person supervision and practice autonomously
  
• Must be proficient with medical instruments and equipment required by the work
  
• Sound understanding of all federal and state regulations including HIPAA and OSHA
  
• Able to show proof of complete COVID vaccination
  
• Ability to drive to patients homes
  
• Must have access to transportation, a valid drivers license and car insurance
  
• Must be able to attend occasional training/meetings at out of state headquarters
  
• Bilingual in Spanish preferred
